[Path of Exile 2] Difficulty settings — do you drop down or push through?

Hoping someone can sanity-check my reasoning before I commit to it. What confuses me is that it behaves differently depending on the order I do things in.

The patch notes hinted at this but never actually spelled it out. Any pointers appreciated, even partial ones.
