Is there a limit on how many codes per account?
Finally worked this out, so here is the write-up I wish I had found. I assumed it was a known issue, but I cannot find anyone else describing it.

The patch notes hinted at this but never actually spelled it out. Feel free to tell me I have missed something obvious.

This is the bit people miss — give it a clean reinstall before assuming the hardware is at fault.

Depends on your setup, but broadly the safe move is the reversible one — you can always undo it if it makes things worse.

Worth checking first: the defaults are conservative on purpose, so there is usually headroom to gain.

Depends on your setup, but broadly it depends heavily on your specific setup, so take any single number with a grain of salt.

The usual fix here is the averages hide the problem; look at the one percent lows instead.

Agreed on most of this, except this changed for me after the last patch, so older threads may steer you wrong.
